Parents Have Options

Cuyahoga County Public Library is interested in demonstrating the effectiveness of its youth programs, but in doing so we want to be respectful of parents’ wishes. We created this page to inform parents about our efforts to demonstrate the effectiveness of our youth programs, and to offer them opportunity to opt-out if they do not wish for their child to participate in these areas:

  • Audio/Visual Recordings
  • Surveys
  • Summer Fun Programs
  • Release and Hold Harmless

Some Cuyahoga County Public Library youth programs may include activities that could be considered harmful. This could include using power equipment/tools, products, or food that could cause allergic reactions or products that could cause property damage. Parents must complete and sign a Release & Hold Harmless Form before their child can participate in youth programs that include these kinds of activities

Audio/Visual Recordings Opt-Out Form

As part of our effort to demonstrate the effectiveness of our programming, we use photographs, video images, and/or audio recordings of our youth programs for, but not limited to, public display, commercial or cable broadcast, radio transmission and web access.

For that reason, we ask any parent who DOES NOT wish for their child to be recorded to complete an Audio/Visual Recording Opt-Out Form and return it to us.

Please note that you should only complete the Audio/Visual Recording Opt-Out Form if you DO NOT want your child to be filmed, photographed or recorded. Please be sure to fill out the form completely, sign it, and return it to the librarian leading the youth program before the first day of the program.

Surveys Opt-Out Form

To determine the impact of our programming, we are asking youth participants to fill out a brief survey. The survey will take approximately 3 to 5 minutes to complete. Information about the program will only be reported for groups of youth. We will not share information about any individual child/teen with other parties. Taking part in this survey is entirely voluntary. If you or your child/teen does not wish to take part in the survey, we respect your decision.

Please note that you should only complete the Survey Opt-Out Form if you DO NOT want your child to take part in our anonymous survey. Please be sure to fill out the form completely, sign it, and return it to the librarian leading the youth program before the first day of the program.

If you do not return this form, Cuyahoga County Public Library will assume you have granted permission for your child to be surveyed.

Summer Fun Program Products

This program will use a wide variety of products. By participating in Cuyahoga County Public Library's (CCPL) summer camps you are agreeing to the terms of CCPL's Release and Hold Harmless Agreement. If your child has allergies, please review the products daily with the children's staff at the host branch.

Release & Hold Harmless

In order to participate in youth programs that could be considered harmful, Cuyahoga County Public Library requires parents to complete a Release & Hold Harmless Form. The Release & Hold Harmless Form must be completed and returned one week prior to the start of the program. The completed form must be returned to the program location.
